As noted above, when ordering ala carte, the basic DDP provides a dessert but not an appetizer. (And you can't trade the former for a latter, either.)
A few restaurants offer fixed-price three course meals, which are allowed on the DDP, but with absolutely no substitutions. In essence, you order from the lower end of the menu in order to gain an appetizer.
And, of course, at fixed-price locations such as buffets, family style meals, dinner shows, CRT, etc., you can have whatever you want without regard to its classification as an appetizer or not.
