Do people make reservations for CS or just go up to the counter and get food? Probably a dumb question but I am new to DDP and ADR's

It's not a dumb question at all.

You just walk right up. One simple hint - look at all the lines before you get in one. Sometimes, a register will have two lines, with the cashier working both sides, alternately. I can't tell you how many times I've seen a line five or six deep, and the adjacent line for the same register empty. Get in the short (or nonexistant) one.
 
Counter Service restaurants do not make ADRs. It is literally first-come, first-served and you will line up to place your order and pick up your food. There will not be a server, and you will have to locate your own table in the dining area(s) in and around the restaurant.

Think of CS dining as a streamlined, high-traffic version of your local Burger King or mall food court.

ADRs are similar to what restaurants in the rest of the country call 'reservations,' except that instead of an actual reserved table waiting for you to arrive, an ADR provides a reserved place in line for the next available table which matches your party size.

In other words, the Disney restaurant will not be holding an empty four-person table for you at 7:20, rather at 7:20 they will place your name on the list for the next available four-person table. As opposed to a party of four without an ADR, who will either be made to wait until all parties with ADRs are seated before being placed in line or who will be turned away completely (if restaurant has booked ADRs to cover all tables for the rest of the evening).

I thought I read somewhere that Plaza Inn and possibly Columbia House at MK recommends reservations. I remember someone else on the boards had ressies at Plaza Inn and were glad they did because there was at least an hour wait when they checked in. I plan on trying to get res. at both these locations if possible.

The Plaza is a table service restaurant that takes ADR's and Columbia Harbour House is a regular counter service location.
