Confused about dessert - help!

Hi...I have read about the DDP but I am a bit confused about what you are allowed to get for dessert at Counter Service (Quick Service) places. When children order a meal from the children's menu the meals come with grapes and a cookie or applesauce. Do they ALSO get to choose a dessert (like a cupcake or brownie) or is the cookie/applesauce that comes with the meal considered their dessert?
 
generally the kids meals are all about "healthy" so the applesauce or other fruit that comes as one of the sides on the kids meal IS the dessert. However, you can ask to substitute a cookie for one of the "sides" that comes with the kids meal. They cannot choose one of the regular desserts, though, it's a specific substitution.
 
I can't remember which places but last August there were some CS places that had prepacked chocolate cake and carrot cake. I remember getting tired of both and mixing them up for my son, so I always walked away with a cake for each of us.
