boy scouts lost sons medical form

that is required for camp. He had a physical spring of 2003 for boy scouts and the dr. filled out the form that is good for three years. Well camp is in TWO weeks and the leader calls me tonight saying CJs form is missing and he needs to have another on file. Okay....except we DON"T have medical insurance for another three weeks when DH goes from 29 day orders to his deployment orders and ohhhh yeah......CJ isn't home between now and than because he's at ANOTHER camp!!!! So taking him in to the doctor because YOU lost it is NOT an option!!! I'm hoping the DR will have something on file and will feel okay with doing another form without seeing him based on the old information. Not that I have ANYTHING else to do over the next few days.

FYI,
Boy Scout medical forms are only good for one year. The dr. should easily be able to complete and date a new one for you....

Class 1 & Class 2 medical forms are good for 3 years, Class 3 medical forms for persons over 40 y/o are only good for 1 year.
I hope you can easily get another form from the doctor. Our doctor will fill out forms if the children have had a physical within the last year. As a leader I keep an extra copy for all of my scouts and I also have the parents keep a copy. When you get another form from the doctor run off a copy for yourself to keep.
I hope everything works out for you and CJ has a great time at camp. My boys love summer camp!

S/be no problem! Brittany had her physical in March. I just mailed a new form for her new care program to the dr's, they filled it out & mailed it back to me.
