Bahamas Immigration Papers

We received our cruise docs for April 14 on the Wonder yesterday and there is mention of a Bahamas Immigration paper that needs to be filled out. I didn't see this document, though. Do I need this? Has anyone else not received this document?
 
This is not a separate paper -- it's a card in your bound documents package. In our package it is the last form before the "Cruise and Airline Contracts" section. It is marked "Arrival and Departure" in the upper right corner. If you don't have it and can't get it before your cruise, I would imagine that there are extras available on the ship.

There is a desk as soon as you walk into the port where they will help you with this document if needed. I am sure they would have extras if you didn't get one.
 
We are on the 4/7 sailing of the Wonder.

They used to send you a separate piece of paper for the Bahamas Immigration, but now they have a page that is included in each of the "Your Travel Booklet" cruise documents/tickets that is supposed to take place of that document. It says Bahamas Immigration across the left top and Arrival/Departure across the right top. It just looks like they have not updated the instructions yet.

You should be all set if you have that page and you fill it out!

I found it, thanks for everyone's help. Next time I'll look with my eyes open!! I hope that I don't miss the Wonder and get on the SS Minnow by mistake!
