Another Documents Questions

Going through our documents I have a question about the form asking for credit card info...

Do you need to fill out one per person, or can just 1 form do....Its me,, DH and DS, and will be all on one credit card...

You fill out one credit card form and list the names at the bottom of the form of those charging on that account.
