This is true in most cases but as my friend's family and I found out this last week, at some of the places it won't show up.

It is my understanding none of the cash registers have the pictures showing up. Only the turnstiles at the front gate. They are supposed to ask for ID with any discount, but I have noticed some do, others don't.

This is correct. Only at the turnstiles will the CM see your photo. Everywhere else - cash registers, restaurants - they don't have access to your picture. So they ask for photo ID (drivers license or equivalent).

And if you've had an AP in the last two years, they'll already have your picture on file. Our earlier APs had expired a year before, so we were able to do everything with one-stop shopping at the ticket counter. Brought our vouchers, got the APs issued, and we were on our way to the turnstile, where our APs immediately worked and our photos appeared on the CM's screen.

I'm glad I found this thread; I had no idea that guest services center in city hall for APs no longer exists!

Does upgrading from a regular or hopper ticket to annual pass now also occur at the turnstiles or the ticket booths?

To upgrade you would need to visit a ticket booth. The turnstiles can't upgrade you, they only give you a temporary ticket if you have a voucher. Since you would be upgrading from a ticket, you would not have a voucher. Any ticket booth will help you, though! Have fun!
