All at once yes. Monthly no. They deduct from bank accounts for monthly.

You can definitely pay them all at once on Am Ex with no extra fees; we do this every year.

You can also put an entire add-on contract on Am Ex, for no extra fees. We were planning to pay cash, but our guide mentioned we could put it on a credit card if we wanted miles or points or cash back (depending on the card.) Very cool!
