Yet another Dining Question...Add on's???

We are traveling to WDW first week on October and have the free dining plan. To make sure that we have PS's at places that I know everyone will eat I have been checking out menu's at various TS restaurants. Here's the question: When I go to a TS restaurant...for arguements sake lets say Concourse Steakhouse and order the Filet Mignon. On the menu it states "Add a skewer of garlic butter roasted jumbo shrimp to any entree $9.99" - Is this an "extra" price to us on the dining plan or included in price of entree. There are a fewl instances of this in various restaurants and just want to be prepared. I am planning this trip for 8 people so need to know answers to ALL the questions :confused3
 
Just back late sunday...we were allowed the add on at Spoodles, but told at Le Cellier that if we wanted to add the lobster tail it would be $12.??. No extra dessert at O'Hana. We were allowed scallops, shrimp, etc. as appetizers at Teppanyaki even though they were listed as sides. We were allowed coffee with dessert at meals. Le Cellier allowed shakes, smoothies, and the fancy coffee drinks- we were told any 2 non-alcoholic beverages. We were allowed shakes at Sci-Fi, but only one drink...not in addition to soda. All other PS were soda, tea, coffee, only...no frozen specialty drinks.
 
We just got back and used the Premium package for all of our dining.
Only visited one place that had "add-on" options available and our waiter told us he treats them as an appetizer, so if you skip the first course, you can have the add-on included (along with entree and dessert).
I don't know if this is the "rule" or just how he decided to handle it. (on this particular occasion, we split a real appetizer and got one add-on which we split as part of the entree).
 

So, you tell the server when you are seated that you are using the dining plan and he/she sort of walks you through what you can or can't order or how it will be billed? Or did you have to keep asking? Or if you ordered something not included, the server told you that it isn't included or there is an extra charge for that?

Can you get ANY entree on the menu (not talking about the Children's menu debate) while using the plan or are there a few restrictions?

Did you travel with children and if so did you always order them their own meal or split food? I am concerned with all that is offered on the dining plan that if I order an app, main course, and dessert for each of us (4 people) that alot will go to waste, so sharing some meals would be a great option. Was thinking of ordering dh a meal, me a meal and 1 childrens meal and charing with 2nd child. Any advice?
 
From what I have read it depends on the place and who you get as a server. If they were to have to make up "Official" rules to cover everthing I am guessing the answer would be no for everything extra. If you get a server that let you get what you are asking for it is probably a bonus.
