There are kiosks scattered throughout each park with each location noted on the maps.

Will that work if you're staying offsite and not buying the Magic Band? What do you give to the cast member to show that you have FP+? Will they know by simply scanning your park tickets?

Yes, you can just scan your park tickets at the Mickey head entrances to the FP+ lines.
