We've had 3 experiences that probably fall under this thread.
First, my wife and I were on an adults only trip (kids were at camp) and we went to Nine Dragons for an early dinner. There was a good sized (what looked like extended) family seated right across the aisle from us, but there was no Mom. Dad sat in the chair like the king of a small country and grown son and GF/Wife felt it was necessary to disipline the young son of (I'm guessing here) sister. The kid was obviously tired and probably hot and probably not excited about eating at a Chinese restaurant so we had to spend our whole meal listening to "if you don't quit cryin, I'm going to take you outside and give you somethin to cry about". And comments from other family members to "take him outside and give him a good one!" It totally wrecked the meal for us. We couldn't wait to finish and get out of there.
Second one was a total accident, but the CM at Matsanoma Lounge dumped a whole sprite down my DS back. He was a very good sport about it (it was hot out that day and he liked the cold). The CM apologized over and over and the manager apologized a couple times. It almost got anoying that they were apologizing so much. They ended up giving us some amount off our bill which was fine. At least is was a clear liquid so it didn't stain.
Third was dinner at CG last year. There was another extended family (of ~12) who were letting their young kids do whatever they wanted. Two of the youngsters crawled under the table next to them where the family had 2 well behaved kids enjoying dinner. These same 2 kids were climbing on the chairs and jumping off. The floor around their table looked like a hurricane had just gone through with all the food, linens, silverware laying around. The whole time the couple sets of parents and grandparents just sat there smiling while the kids did what they wanted. It obviously made a number of tables not enjoy their meals like you hope to expect at a place like CG. Thank goodness they were 3/4 way done with their meal when we sat down, so at least we got to enjoy most of our meal.
