It depends.
We eat pasta dinners (with or without meat) 3 days per week so with those meals it's with salad or veggies and no other starch/carb. Kids (the older one's) are athletic and active (ie ride bikes, martial arts, etc) and I exercise 4 or 5 days a week so I make sure we eat enough carbs for energy.
Twice a week we eat vegetarian meals which includes legumes or a protein source, veggies on the side, plus whole grains and a salad.
The other 2 days are week are usually a meat or ethnic dish (like a stirfry or chicken cooked Asian style) plus brown rice or couscous or other whole grain and a salad or veggie will be eaten as a snack if those foods are too much for us.
Generally, fruit is reserved for snacks and we try to eat a little protein with it like cottage cheese, breakfast and lunch usually has more carbs/starches, and we eat treats like ice cream or pie 2 or 3 days a week only.
