I just found this form by accident on the Southwest site and thought I would share it here!

It is a form to present at the airport describing your wheelchair and what they need to know.
It does not replace the instructions you should attach to the chair to explain how to load it on the plane, but it will probably be helpful for them to know all this info - for example, I had not thought of putting the total weight of the chair on the form, but that makes sense, especially as mine weighs almost 300 lbs!!
