Everyone has to fill in a green visa waiver form. Then you have one white form per family which is the same surname one. I don't think children would have to hand one in to customs to be honest. I wouldn't worry about it, you could always just fill in two on their behalf and hand them in.

Related Question:

Is there anywhee you can get the forms in advance of flying?

I always have trouble trying to fill these in on the plane and usually end up having to re-do them twice. :(

Unless someone has brought a large number of spares home then the best bet is to go to an airport that has trans-atlantic flights and see if they have any there.

Usually they give you the forms when you check in, so make sure you ask for plenty of spares :)

We were given them when we handed the cases over in the morning at Manchester. I filled 3 of them in on the plane and got my then 18 year old to do his own (as he is flying back by himself a week on Saturday). He made a mistake and the steward just gave him another couple. So chill, there are plenty of spares if you do make a mistake just make sure they are right before you are off the plane.

We did see someone get sent back to do the form again at Immigration at Orlando Airport on 1st August. He must have been gutted because he was straight in to see the Immigration Officer because we seemed to get off the plane quickly.

Actually the reason I asked is that on my last flight (Gatwick to MCO) they wer very short on them and we ended up havig to hand over one that was wrong (but luckily got away with it).

Maybe they just keep more forms on the Manchester flight but they've always said they had a "limited supply" when I've flown. It's just a pain in the butt.

When we flew through LGW in July, the check in desk had piles of them sitting there.
We weren't offered them and I only saw them at the last second so keep an eye out for them when you next travel.

We fly VA and do the twilight check in and i pick the forms (and spare ones) up there and fill them in at the hotel.;)

Same here we picked them up when we done twilight check-in with VA, it was so much easier then filing them in on the plane.
