Indiana, but I've also lived in New York and it was the same way there.

My FIL was a chef who worked in Maine, New Hampshire, Massachusetts and Florida and he never heard of such a practice.
Trail's End did and it's a buffet.
The chef made me grilled chicken breast, actually he made me two hence the need for a to go box(made a great sandwich on allergy safe bread to take to the airport on our departure day. Airports suck for people with food allergies.)
I asked if it was okay to take the other piece to go and my server didn't even hesitate.
I think for the most part it is just the ability to get a to go container. Trail's end has a connected 'to go' counter so they had containers.
Brown Derby has NO to go service but they had no problem getting me a container to take our deserts in (was on DDP but we don't typically eat dessert after meals). So even restaurants without to go services had to go boxes.
At Casey's I ordered corndog nuggets to take back to the resort for lunch. No to go containers. Just wrapped them over with a napkin and got a bag from the store next door, which actually worked well as the napkin absorbed any condensation and the bag was easy to keep the top open so that they didn't steam like they would have in a container. If I'd known I was going to grab CS I would have brought baggies from the room but it was impromptu. I haven't ever been able to get a to go box at a CS.
It seems this was a unique situation. The chef cooked something specific to meet your needs. I wonder if you didn't take it as a "take out" would it have had to be thrown out? I bet yes
