Terms and Contract for Renting Points

For those of you that have rented points before, what are the terms that are most commonly used when it comes to the money. Specifically how much do you request up front at the time of the reservations and when is the balance due? What type of limitations do you put on cancellations and any fees that may be involved? Any other suggestions would be appreciated since this is my first time at renting points.

Is there a standard rental contract that is used and could someone recommend where I could get a copy of one. If there is one available online somewhere?
