Tax Question: NJ Public Employees' Retirement System

Statement of Allowances and Deductions.

I'm trying to do my mom's taxes and she has a statement from the above. It lists her current earnings (supplemental and cost of living medicare part B) and deductions (income tax). There is no form number. Do I have to list this anywhere on her taxes?
