Someone please help who has photoshop experience

OK so I'm planning my dd 3rd birthday party and i'm going with the Princess and frog theme. I want to make a fake movie pass for the kids, laminate them put them on a lanyard so they could wear them around their neck. I found a movie pass but I need it to be blank ( without the other kid name or information on it) Can someone PRETTY PLEASE help me? I would really appreciate it. If I have to beg I will

Here is my attempt. Since the words are in the movie poster design, I had to "re-create" the pass. Cute idea!
