Can someone please tell me what exactly you get with this, the difference between the two and after you buy the box, do you still have to pay for movies, tv shows, etc?

It depends on the service. Most people buy these to work with Netflix. Netflix is a subscription service, so yes there would be a monthly fee. The cheapest you can do with Netflix is $7.99 a month for unlimited streaming and no DVDs.
I'm debating on which box to get as well. I picked up the Roku because it was the cheapest, but now I'm having second thoughts because you can't play media over the network. This box is for DD's TV since I have an HTPC hooked up to the main TV. So far we have not used Netflix that much, but I think we will since we can expose DD to many different shows with it. I'm trying to ween her off of Barney! She gets fixated on something and won't let it go. For example, I started introducing her to the Disney films, but now we are stuck on Mermaid.
For us, we have a problem of too much content. The HTPC has an antenna for local stuff, Netflix, Boxee, Hulu, Microsoft Internet TV, as well as having all of our DVDs ripped to a hard drive. I have also ripped all of my CDs (1,000+) to a hard drive so we can have our entire music collection easily accessible.
