What restaurants require prepayment?
What restaurants have a if you don't cancel they charge you $10ish fee?
What DTD restaurants can be booked directly by calling the restaurant?
What is the cancelation policy of prepayment and no cancel = fee places?

Prepayment? As in, pay when you make your reservation? I've never heard of that, so I'll say none.

As far as needing a cc# and being charged $10 if you don't cancel, I think it's all DLR restaurants in the parks that take reservations. I know we booked Tortilla Joe's ahead of time via Disney, so they have that policy. I'm guessing that any of them that you can book thru Disney have that policy. HTH!

I do not know of any that require pre-payment.
All Priority Seating have the $10 cancellation.
ESPN, Rainforest Cafe - I have made reservations at both these directly through the restaurant.
Cancel more than 24/hrs & you are not charged the cancellation fee.

You can book Ralph Brennan's Jazz Kitchen on their website. They link through to OpenTable, so I suppose you could book directly on that website as well.

Btw, the $10 no-show fee is per person on the reservation. This is for any dining booked direct through Disney for restaurants in the parks, the onsite hotels, and downtown Disney.
