Renting question

Not sure if I'm in the right board but,

I rented points from a DVC owner and after paying half they issued me the reservation from Disney with my name and beneath that it says, "address required upon request..."

How does this work?

That's perfectly normal. At check in you will have to show a photo ID and give them a credit card if you want to be able to charge to your room. You really won't even need that reservation form but it's nice to have.
