Photo dimensions different than settings ?

I have a Canon S2 IS. I have it set to take photos in the largest size (2592 X 1944), yet every once and a while it will take a photo at a small size (260 X 195). What is causing this to happen ?

That is not even a setting on the S2. Is it possible that you are looking at a thumbnail image somehow? Is it an image of the beginning of a video by any chance? If not, you could have a defective camera.
