Personalized Menus/Diver Sign

We are surprising DD with a birthday trip in a couple of weeks. I ordered a birthday cake for her actual birthday night (dinner at HDDR), but forgot that some places do personalized menus. Do they do menus (or something else) at:

Coral Reef (diver used to do signs...still happening?)

San Angel Inn

The Wave

If any of the above do something special, how do I go about contacting them? Thank you!

Coral Reef's personalized menus and divers with signs were discontinued in 2009.

None of the restaurants do menus with pictures like Coral Reef used to do. Some of them can print a menu that says Happy Birthday on it, ask at the podium.
