Passport help!!! Please read!

Thanks for reading. Many years ago, around 23, I had a passport issued in my maiden name. I have lost this expired passport. Now I am trying to apply for a new passport using the government's website to create the application. They require the old passport's number and issue date. I can't create the form. What should I do?
 
I think some where I read if your old passport has expired more than 15 years ago, to go ahead and apply like you have never had one.... I will see if I can find that info and link you

I had an older passport with stamps from Communist Romania. No way was I going to give that up. It is expired anyway. I checked the box that it was lost - which it kind-of is -somewhere in my storage closet in the basement. Got my new one just last week.

Yes, there is a place to mark that it was lost, but if I don't put in the number and issue date it will not create the form that I need to take to apply for the passport.

You need a DS 11 http://www.travel.state.gov/passport/forms/ds11/ds11_842.html

And then the DS-64 for lost or stolen passports... http://www.state.gov/documents/organization/79958.pdf


It says fill it out as completely as you can... since you dont remember your passport number, you dont need to fill that out. I would try to remember as many details as you can, then ask the Passport application site what they think you should do. You could also call the Passport office and find out on Monday


edited to add, I see your using the website to fill out forms.. I just printed my out blank and hand wrote them, maybe you can try that?

You know, they do return your expired passport, they just mark it EXPIRED.
 
I got my old one back with a hole punched in it...
 
Don't worry. I had the same problem. Mine expired about 10 years ago and I'm pretty sure I destroyed it. When I filled out the paperwork, I just typed in unknown in those boxes and my passport came through in 10 weeks with no problem at all.

That is exactly what I just did, I applied as if I never had a passport before. I have an old expired passport from High School, I have no idea where it is or what the passport number on it was. My new passport just arrived today.

Was your passport issued more than 15 years ago? If so, answer the following question on the Passport Application (DS-11) as below:

If not, you will need to find the passport number for the application or just state it as lost and fill out the form DS-64 (Statement Regarding Lost or Stolen Passport) to accompany your Passport Application.
