He may have legally changed his last name to Cruise, but since his middle name is legally Cruise, than I don't think he is Thomas Cruise Cruise. I think he just dropped his last name for a better stage name, but his Mapother name should still be on this BC. Should the birth place of mom and dad be listed? Not just the state? Boy Massachusetts BC have oh so much more info.
The birth certificate is correct in its look, I just pulled out my daughters who is also born in Los Angeles County. As far as my information it just lists HI as my state of birth, not the city. The delivering doctor signed as the Attendant or Certifier and lists a license number. I signed the BC the day after her birth, the doc signed it 7 days later and it was accepted into the registrar 22 days after her birth.
In order for me to obtain a copy I needed a photo ID matching the name on the birth cerificate. I am wondering how a copy of Suri's BC ended up in the hands of a tabloid.
I just looked up what is required to get a copy:
*The registrant or a parent or legal guardian of the registrant.
*A child, grandparent, grandchild, sibling, spouse or domestic partner of the registrant.
*A member of a law enforcement agency or a representative of another governmental agency, as provided by law, who is conducting official business.
*A party entitled to receive the record as a result of a court order, or an attorney or a licensed adoption agency seeking the birth record in order to comply with the requirements of Section 3140 or 7603 of the Family Code.
*An attorney representing the registrant or the registrant's estate, or any person or agency empowered by statute or appointed by a court to act on behalf of the registrant or the registrant estate.
*Any funeral director that orders certified copies of a death certificate on behalf of any individual specified in paragraphs (1) to (5), inclusive, of subdivision (a) of Section 7100 of the Health and Safety Code.
