My wife if currently battling stage 4 breast cancer. We are trying to plan a bucket list trip to WDW sometime in October.

She currently needs a cane for walking long distances, and large amounts of stairs are an issue.

I'm just wondering if a DAS is something I should look into, or if a cane is something the various CMs will just address at the attractions as needed?

I think your best bet is to rent a scooter for her. The parks are huge, and if walking is a problem, she'll appreciate the scooter, and probably have a better time since she'll be able to do more. There are a few places to rent from, the ones at the parks are kinda expensive from day to day.
 
If all she needs is to use the cane, then that will be handled through the standard queue (either standby or FP+) and by CMs at attractions with obstacles like stairs.

The DAS is for those who are unable to wait in a standard queue environment and therefore requiring waiting elsewhere. If your wife has, or develops, other needs -- feel free to discuss those with Guest Relations. But mobility and stamina needs to not require a DAS.

The DAS is for people that can not be in the standard line and need to wait out side of it.

If the only thing she needs is a mobility device the CM can see this and will let her by pass the stars.

there is a lot more walking at WDW then most people think and she may be able to get around just fine at home with a cain but she may need more then that at Disney I would have a few fenders for wheel chair or ECV just in case she needs the extra help.
 

I think a scooter is her best bet, but also be prepared if you need to rent a wheelchair to help her along. Depending on treatment and how she is feeling, manipulating a scooter may be hard for her come October.

We're vets, so we know the walking involved. But she is a trooper and is hoping the cane will be enough. In the event its not, I'm assuming ECVs and wheelchairs need to be reserved well in advance?
 
Not always. You can usually rent same day from Disney, and even sometimes, from outside vendors. I would suggest doing your homework before leaving, and then have the phone numbers for a couple of outside vendors. This is just in case Disney is sold out.

I would rent from an offsite vendor instead of renting each day at the parks. This is for two reasons:
1. Renting at the parks is first come, first served. There's no guarantee they won't run out on any given day.
2. There's a lot of walking before you ever set foot in a park, those distances will be just as exhausting for her.

3. Disney Springs is HUGE now; if you all go down there, I feel certain that the ECV will be needed/used.
