Need help with "address" book

My daughter is attending a 2 week summer camp. I'd like to make her (and possibly her cabin mates) some type of notebook/altered file folder to jot down everyones name, email, facebook and phone number (gone are the days of real addresses). Any suggestions?

how much work do you want on this???

simple - check your computer(Works, Word) for this already to print then add to small 3 ring binder notebook

Really crafty?? - can you make word books?? then they could add a picture of the person later (on one side)

Somewhat crafty...I own a Cricut.

Thinking maybe cardstock folders with pre-printed insides from Excel to fill in the blanks? I just ordered the Lifes a Beach cart so I can do custom covers or embellishments on them. I figured I mail them to her for the second week of camp when they can exchange info before leaving.
