There are 3 monorail loops and they all connect at the T&TC (ticket & transportation center).
Resort Monorail - Runs clockwise around Seven Seas Lagoon
Stops: T&TC, Polynesian, Grand Floridian, MK, Contemporary
Express Monorail - Runs counterclockwise around Seven Seas Lagoon
Stops: T&TC and MK
EPCOT Monorail
Stops: T&TC and EPCOT
MK to EPCOT:
Take the Express monorail from MK to the T&TC then switch to the EPCOT monorail at the T&TC station. Follow the overhead signs. It's easy.

No, as noted above the resort monorail stops at the Poly and proceeds to the MK via the GF, so there's no need to go to the TTC to catch a monorail to the MK if you are at the Poly.
It is necessary to go to the TTC to catch a monorail to Epcot, however, but the TTC is just an 8-10 minute walk from the Poly. You could ride the resort monorail all the way around from the Poly to the TTC, but you will stop at the GF, MK and CR before you get to the TTC and it will take much longer than walking.
No matter what Longhouse you stay in you can walk to the TTC. Some might be faster and there are others which would take about an extra few minutes, however they are not bad at all.

We stay at the Poly every trip -
When we are going to the MK we catch the monorail at the Great Ceremonial House.
(Keep in mind you can also catch a boat to the MK from the Poly.)
When we are going to EPCOT we walk from the Polynesian to the T&TC to catch the monorail. The Tahiti and Rapa Nui longhouses are the closest to the T&TC but we've stayed in the Fiji longhouse, which is one of the furthest longhouses from the T&TC, and the walk is not bad. I'd say no more than 10 minutes.
