Yes, it'll be extra. The only included (free) beverages at dinner are coffee, tea, soda, tap water and milk. My DDs had a few smoothies at dinner in July and they were $3.95

I just want to add that is only fountain soda's that are included not the cans. My DS asked the waitress what soda they had she named all of them including the one in the cans, he picked Root Beer. It came in a can and that we would get charged
Only at the end of the meal we got a bill. Same thing happen to our table mate their DD ordered a smoothie, the waitress did not mention once the extra charges.
I think they consider a milkshake a smoothie...same item different names. You can get for example a chocolate smoothie which they would bring if you asked for a chocolate milkshake and you would be charged.
We were told by our dinner server in 2009 that they are not actual milkshakes because they use ice instead of ice cream. He said that all ice cream on the ship has to be made - there's not enough freezer space to be able to store it all onboard - so they use ice in all the shakes/smoothies. I dont know if this is actually true - I've never tasted/ordered a shake. (This was told to a tablemate at dinner.)
On our cruise in January our friend asked for a milkshake for dessert and our waiter made one for him. He was not charged for it. As far as chocolate milk, I think they have cartons of it on the buffet, so I guess they have "real" chocolate milk at dinner. On my RCL cruise a few years ago I asked for chocolate milk with my breakfast and it took a really long time to get it. I think they used hot chocolate mix to make it, judging from the looks of it.
On the Dream they have small individual cartons of chocolate milk at breakfast and at dinner I saw our asst. server pour my sons drink from the carton as well.
When we went on RCL (Mariner and Freedom), they also had the indiv. cartons avail. at breakfast and I think dinner as well.
On our Disney Dream cruise a few weeks ago we were leaving our room (deck 5) when a CM dropped something off his cart right outside our room. The box said something chocolate so that immediately caught my eye, and I asked what it was and he said it was all ingredients for making milkshakes in the teen club (don't remember if he was referring to the vibe or edge).
I think on Disney if you were to order a chocolate milk from a bar that it might be with syrup and I think you may have to pay for this like you do soda's from a can from the bars.
