Looking for itinerary template?

We will be going to WDW in 2 weeks and I am trying to compose our itinerary onto large index cards but I'm having trouble locating a template of sorts! I have seen a couple of people make their own and was curious if someone had one I could use to fill in our info? Thanks!!

Index cards are a good idea. Hmm...might have to figure this out myself
