I rarely spend more than $10. Because I have all boys, I tend to buy things like small R/C cars after Christmas & on summer clearance for around $5. I stock up on them for birthday parties. Even adding batteries I can stay around the $10 mark.

For new/first Grandson's first birthday
DH and I saw a ride on (scoot with your feet) Thomas the train with sounds and bought on the spot. It was only $50 and he adored it!!!
About $30. My 2-year old just had his first preschool birthday party, and we bought them a hardback book, a Spiderman art set, and a card. I think it came to $32, which seemed about right.
We do $20-25, and maybe a little more for *best* friends. But my kids are older now (12 and 10), and the invitations have slowed down a bit. I definitely shopped sales/clearance for gifts closer to $10-15 when they were getting those whole-class invitations a couple times each month.
$10 for a friend and $20 for family. The boys just went to their cousin's joint birthday a couple of weeks ago. I bought both boys Iron Man hands for pretend combat. They were $20 each at Kmart and I had some surprise points.

My son had friends over for his 14th birthday last night. He got 2 cards with $20 in each, and the rest just walked in and gave him $20 cash. I felt like we were charging to hang out at my house. LOL
Honestly, it depends on the kid. Back when my son was little I topped out at $20 for friends gifts with the exception of best friend. Now, I'm all about what the kid will like. We have a birthday party for 2 kids (siblings 1 and 3 years old) this coming Saturday. We spent $10 on the 1 year old but she will LOVE it. And for her bother we spent a little over $40 and he will also LOVE it. It isn't about the dollar amount. It's about how much the kid will enjoy the toy.
