Yes I have one. I just got back from AKL the other day.

I'll try to scan it into my computer tonight and insert it as an image of both sides...

It looks as though I can't embed an image directly from my desktop, unless I'm missing something, so I'm uploading the scans to imgr. This will be a test as I'm not sure which link format these boards likes...
