The main reason they need to know your travel agency is because IF anything happened on the islands it would be under the jurisdiction of that island's government and not DCL, so they would be better able to find the details on you and your emergency contacts, etc. Although DCL does have your emergency contact info, they generally do not know as much about each passenger as a personal travel agent would, especially if someone booked with a local agent in their home town and not a virtual online business. Also, regarding terrorism, it would allow them to easily track your method of payment and who paid for the cruise. That is important if a security threat arises, and is just par for the course for travel nowadays. Airlines are not the only ones "tracking" us.
As for putting incorrect info on the form - If it was a deliberate act, one could get in very big trouble for that. DCL DOES check these forms at the terminal and if they are incorrect they must be redone.
