Yes, you will see people falling down drunk and intoxicated minors. If your cruise goes to Mexico or a number of other Caribbean destinations where either the drinking age is 16 or simply not enforced you'll see plenty of teens come back on board staggering down halls.
Some cruise lines are more tolerant of this type of behaviour than others. I've cruised DCL and RCCL. On Disney we saw no one drunk. We saw adults drinking, but in moderation. It was definitley a family atmosphere. We saw no drunk teens even after shore excursions.
On RCCL it was so different. They push the alcohol to the point of it being ridiculous. We saw intoxicated people in the middle of the day. We saw intoxicated people at dinner. And the couple times we were out of our cabin after dinner we saw a LOT of intoxicated people all over the ship. My son was 19 at the time, and we had NOT given permission for him to consume alcohol, yet bartenders offered it to him (and he's young enough looking that he still gets carded to get into "R" movies). We saw a LOT of teens come back onboard after our stop in Cozumel drunk as skunks, one vomited in the hallway a few cabins down from us. We were very unhappy with our RCCL cruise for a number of reasons, this being just one.
This is one of the reasons I'm leaning toward going on a Disney Cruise. I know people do drink on the Disney ships, and I fully plan to be one of them. I just don't want the "spring break" atmosphere that Ducklite described.
Also didn't see it on RCCL's Radiance class in Alaska last year. Every ship is different... but think the newer/larger ships in every line are more family oriented.
