Several people have recommended http://www.easywdw.com/waits/ as a good resource. I have it bookmarked on my home screen. There are no images so it loads quickly, which will help a lot in the parks.
We use MDE--but it is known to sometimes not have the wait time information. At least in our experience first week of December, last day of Osbourne lights at DHS, and last Saturday at MK.
Not sure the issue--but it does sometimes come and to whether we use wifi at Disney or my own (whatever the iPhone term is to use just data versus the wifi).

It should be accurate, but I have heard the App can be quite slow and have trouble loading in the parks. Plus I've heard a rumor that they pad the wait times at the end of the night to discourage people from getting in line. But that could just be a rumor.
I was wondering the same thing. The "Actual Wait Times" from the 3rd party sites have not been working for over a week now. I'm pretty bummed, I had planned on using them for our trip in May. It was nice to be able to tell the difference between the posted wait times and actual wait times, especially when the posted wait times are so inflated these days.
I just checked the MDE app against the Touring Plans app, and the TP posted wait times matched up exactly with the MDE wait times for the handful of attractions I checked out. Not terrible scientific, I'll grant you, but it would appear that TP still has access to posted wait times.
