How would I go about having a pin made?

If this is in a FAQ somewhere, I apologize. Search is down at the moment and I'm just curious about what vendors anyone here on the DIS has used to have a custom pin made. I know that there are literally hundreds of folks out there who manufacture custom pins, but I'd like some DIS references.
