Help with Coral Reef Personalized Menu?

Hi,

Would anyone mind posting a picture of the personalized menu they received from the Coral Reef? I am planning one for my sister for her birthday and was told to send 3 pictures that I want included and what I want it to say.

I am the most uncreative person and thought that if I had an idea of what the final product looked like, I'd have a better chance of thinking of something good!
