help- need coupon from April DC newsletter

I don't know where to post this but, I figured you guys would know what to do. I finally broke down and bought my 2 DS the 102 Dalmations video. I know there is a $5 rebate through DC but, I don't have the form anymore. I checked at the DC site but, there isn't a printable form. Any chance any of you have one that you didn't use??? Any ideas???
