Help! I lost my itinerary form

I've been using it for years. Somehow dumped it when I was cleaning up files in Microsoft Publisher. Each trip I just put in the dates, show times, PS's and park must see attractions and print it out. When I went to update it today I couldn't find it. I loved having it all on one piece of paper. Does anyone have a park plan they would'nt mind sharing with me? If not I guess I'll start from scratch.

I have one that we use. It reduces everything down to one printed page. It is in Lotus format. You would obviously have to modify it, but its a good start if you would like to take a look at it.
