Filling out documents question

My husband and I are cruising with our four kids. Two of the kids are staying in grandma and grandpas room. Do I have to fill out this form for kids not traveling with their parents? They are traveling with us just not in our room.

fanzz, that's just for parents that will not be on the boat with their children i.e. grandma and grandpa are going with the grand kids. You're fine without it.
