Filling out documents......HELP!

Could someone tell me how to fill out the pg. with credit card charging info? My cousin wants her teen to be able to charge to her cr.crd. so when filling out her own documents she listed her dtrs. name on line where you put names of others who can charge to this card, but what does she put on her DD's document?Does she put her name and cr.cd. # on this document too?
Also, on other 2 cruises we had a separate white paper to fill out for immigration but it wasn't in with my paper work from DCL time time. I did see there's a pg. for immigration in your doc. pkg. . Is this taking the place of that separate paper now? We have cruised 2xs before and I don't remember being so confused about the paperwork......must be getting old!

You are right in that if she wants her daughter to be able to charge to her card she should put her name on the credit card paperwork. As for the daughters paperwork that one (the credit card info) doesn't have to be filled out or turned in. They would only have to do that if they had separate cards and wanted separate billing.

Not sure about the imigration paperwork. For our 4 day cruise it is a page in the book. Can't remember what we had last time on our 7 day cruise. Hopefully someone else can help you on that one.

You are correct that the Bahamian immigration form is now a part of the document package and not a separate piece of paper now. This one threw me for a loop too. I called DCL and told them that they forgot to send me the immigration forms. The nice CM told me that they were now a part of the bound pages.
