All prebooked first, then more one at a time. You may find the FP FAQ helpful, where basic questions such as yours are answered.

All three must be used before a fourth can be booked.

You need to use all 3. Immediately after you scan your third FP+ get a new one. Keep refreshing the app to get a sooner time.
