The fastest way to get a good resonse from Amazon, in my experience, is to go to your Orders page and click the "File/View claim button" for the order in question. I never have good luck with chat or phone support, but every claim i've filed has been taken care of within 24 hours.
I'm pretty sure they outsource the phone/chat to regular people working from home, not in a huge call center, so their ability to get things done isn't nearly as effective as the people who handle the claims.
