If you aren't comfortable about giving certain items of info then let the school know that but really in the time it took to go through this whole thread and post you could have filled out and returned the form. Just fill it out to your level of comfort I mean it's going to your children's school, not a door-to-door salesman or mysterious phone call. I was surprised when we qualified for reduced lunch at my daughter's school which isn't totally relevant b/c her school gets universal free lunch and breakfast. It's a good thing too, I substitute teach for the district and I know that there are children who almost only get to eat at school. It's also very important for other funding as some posters have pointed out.
