Documents/Info needed for Niece travelling with us?

We are taking our 7 year old niece with to WDW for the first time this summer.
What type of information should I have her parents give us to take along. As far as I know, she doesn't have any type of photo identification or a passport. I know we shouldn't have any problems, but I don't want to forget something basic like her medical insurance information.
 
When we took our nephew and niece, we had their birth certificates and letter from their Mom authorizing consent for medical treatment. Thinking back on it, we should have had that letter notarized. Thank goodness, we didn't need that letter!
 
You might want to get a notarized letter from her parents stating that you have permission to make decisions for her in case her parents are unreachable at the time a decision needs to be made.
 
I'm taking my six year old niece to Disney this May and I am bringing a medical consent letter, a copy of her birth certificate (not the original) and a photocopy of her parent's insurance card. For the medical consent letter, you just need the parents to write a short letter giving you permission to consent to medical treatment for their daughter if needed. I'm also having mine notarized. Not sure if this is totally necessary though, maybe someone else knows for sure? I just figure better safe than sorry.

Other than that I don't think you need anything. I flew with my 3 year old nephew and he has a different last name and everything and I was never asked for anything, let alone any kind of photo ID. When I called the airlines to ask what to bring, they said a copy of the child's birth certificate, but no one ever asked to see it.
 

My DD4 travelled with my folks to WDW last fall and last summer she stayed with them for a week while our DD(newborn) was having surgery.

We typed up a letter that stated they had the right to make medical decisions for her in case we weren't there. We also gave them copies (front and back) of our insurance cards. The letter was notarized.

I travel quite often with my DNeph (7) and usually just take copy of his birth certificate and copy of sis's insurance card with letter giving me permission to supervise medical treatment.
 
The different last name won't be an issue. It's so common in families now. I know my son and I have different last names.

I would get the medical consent letter and it's best to have it notarized.
