Orlando airport hasn't been using them for a while, though I did hear for a while that some airlines were still giving them out for whatever reason - perhaps just finishing up their stash, but people were getting to Immigration and the officers were ripping them up. That was a couple of months back though, hopefully even that has stopped by now

Don't know if there is a copy to view, but from what I can remember it's pretty straight forward - one per family. Basic questions regarding how much money you are taking into country and food stuffs etc..We were given two on the plane in case of mistakes, but like I say it was straight forward.

I think the airlines on this side were trying to dispose of their stash whilst coming across as being environmentally friendly, i.e. send the forms on the plane and get US immigration to dispose of them.
