Disney lanyard name tage template needed

Help!! Last year I printed out really cute name tag templates for my kids to put in their Disney Pin Lanyards. Now I would like to do the same for a friend. I only saved the pirate border templates and she only has girls. Now I can't find the website I used. Can anyone direct me to the website. Thanks,
