Disability pass question

I went to the Disability forum but it looked like it was all for Disney, so I am asking here.
My youngest has always gotten the DAS pass, and it has saved us (and you all) from having to deal with him in line. He even qualified with the changes to the DAS pass recently.
We are looking at going to Universal and the application for the pass.

Anyway, I have looked at the application and they want a medical note. That is fine, I am just wondering what needs to go on the note? Is it just why the medical professional feels he needs to not wait in line? It shouldn't have the actual diagnosis on it, right? I didn't know if I needed to upload his evaluation and diagnosis or what exactly they were asking for.

And how far in advance should I do it? I know it says up to 30 days prior to our visit, but if I request it 5 days in advance, is that enough time? That is more what I am worried about. How long does it take them to process the request?

It can show diagnosis. All they use it for is "proof" of disability. I can't comment on how long it takes because I did mine when it first opened but I would do it as soon as you have the documentation. I even just sent my Dr a message through the patient portal and they sent me back a 2 sentence letter that I submitted. It really doesn't require much, just a medical professional confirming a disability.
 
The process needs to be done at least 2 days (per Universal’s website) before your visit but can be months out. The company that does the checking for Universal validates the holder for a year.

I put my details in and had my temporary card in my email before I could log in. It took about 2 days until I got my full card.

Ok, thanks. I wasn't sure what to ask them to put in the letter. So, is that 2 sentence letter the only thing you submitted? No other documentation, or did you upload a diagnosis as well?

I’m a different poster here, but I didn’t even ask my doctor for a letter and just uploaded my diagnosis report/paperwork. It was accepted no problem. Most likely documentation you already have will be fine without getting an extra note. The IBCCES is only confirming the disability exists, it’s universal that will reach out to determine exact accommodation needs after disability is confirmed.

Just the letter, nothing else. They are literally just looking for a medical professional confirming a disability.

Ah, good to know. I didn't realize Universal would also contact me.

I also just realized that his paperwork with the official diagnosis on it has a different name because it was pre-adoption and while he was still in foster care. (he always went by his middle name so we moved his middle name to his first name and then gave him a middle name and our last name).

So that won't work. But I can get his therapist to write a simple letter I am sure.
