When did Disney remove appetizers from the deluxe plan table services?

2014 Deluxe plan: PDF
2015 Deluxe plan: PDF

Both still list an appetizer on page 4. If, however, you're referring to the "regular" dining plan which included an appetizer and gratuity when it was introduced in 2005, that went away in 2008.
