Did anyone get a survey???

I got a survey in the mail today. It looks sort of official but the website it says go to does not. Just curious if anyone else got one and if it is A-OK. It is from Business Insight and Improvement Field Operations.

Yep. Got one back in September after our August vacation. It will take about 15-20 minutes because many of the questions require a typed answer as opposed to a rating.

Received one in the mail yesterday. Just back from BCV on the 2nd. Just finished the survey. It took a while, but I love surveys!
